Karen achieved major recognition in the 1980s. She became Playboy's Miss December 1984 and was named Playmate of the Year for 1985. She was the first Puerto Rican to hold that title. In March 2008, her daughter Nikki Majors appeared in the magazine's Playmate Daughters feature, showing a strong family resemblance.

Karen Velez passed away on July 2, 2023, in Downey, California. Her death was suspected to be from the flu. She was 62.
